Danielle Cackett says there were a series of failings at Colchester General Hospital before her son MacKenzie was diagnosed with a brain tumour and then a tumour on the spine.

And as figures show hospitals are dealing with more patients than ever, 15.1 million people, a senior emergency doctor warns of an impending crisis in A&E and says she has serious fears about the safe care of patients.
