Last month, a serious case review into the death of two year old Keanu Williams concluded that there were a number of significant opportunities to save him from being beaten to death by his mother. It's the latest in a series of horrific child deaths that have shocked Birmingham and exposed shortcomings which have led to the city's children's services department repeatedly failing inspections and the city being branded a 'national disgrace' by the head of the watchdog, Ofsted.
