Ambulance crew Johnny and Fraser get a call to a GP surgery where it is reported that a man in his thirties seems to be having a heart attack - a highly unusual emergency.

In this episode, we follow ambulance crews from Edinburgh and Glasgow and the special operations response teams from the east and west of Scotland as they respond to 999 calls and provide emergency medical treatment.

In Glasgow, Stevie and his team are called to a car crash on a very rainy day, while elsewhere in the city, ambulance crew Johnny and Fraser get a call to a GP surgery where it is reported that a man in his thirties seems to be having a heart attack – a highly unusual emergency.

In Edinburgh, Colin and Dan check and treat an elderly lady with an unusual combination of symptoms. They need to work out what is going on and arrange for her to go to hospital.

In Greenock, another elderly lady has fallen at home and needs to be carefully moved and checked by Brian and his special operations colleagues. Back in Edinburgh, Moira and Blair get a call to a man with breathing difficulties who seems to be having a panic attack that is making his symptoms worse. They need to try and calm him down before it’s safe to take him to hospital.

Elsewhere in Edinburgh, ambulance crew Stephen and Gwion face a challenging situation with a patient who needs treatment but who, because he is in palliative care, refuses to go to hospital. In Fife, a young man has fractured his ankle while walking on rock by the coast and needs to be rescued. Ali and Emma from the special operations team go to help.
